Records are maintained for births and deaths that occur in Winnebago County after 1916. Certified copies of these records may be requested by eligible recipients for a fee. For records of births or deaths that occurred before 1916, visit the Winnebago County Clerk here.

Who can order a Birth Certificate?

  • The person named on the certificate if over 18 years old
  • A parent listed on the birth certificate
  • A legal guardian or representative with supporting court documents

Who can order a Death Certificate?

  • For a death in the past 20 years:
    • Only the person listed on the death certificate (informant or parent) can receive the death certificate.
    • Those with a legal purpose for obtaining must provide proof
    • Death certificates are not public records
  • A funeral home
  • An executor of the estate

GETTING A CERTIFICATE

In Person:

Bring the information you will need and come visit us at the Winnebago County Health Department. Look for the signs for “Vital Records” on the ground floor all the way down the hall and to the left.

Our Address is 555 North Court Street. We have plenty of free parking onsite.

Electronically:

Click on E-Pay for electronic ordering and payment. Then email a copy of your ID and signature to vitalrecords@publichealth.wincoil.gov or mail the copy to the Vital Records department. Birth Certificates ordered through E-Pay will not be processed until a copy of the requestor’s id is received
