The School-Linked Health Center has been providing services since July 2002 and works to promote the physical and mental health of the students and their families.

Our goal is to help children get the most from their education and develop healthy lifestyles.

The School-Linked Health Center is available through an Illinois Department of Human Services Grant awarded to the Winnebago County Health Department. It is a cooperative effort with the following agencies:

Eligibility	Any child is eligible for medical services from the Rockford School District #205, Winnebago County and surrounding areas, as well as residents of the Blackhawk Housing. We serve infants and children up through 18 years of age. Mental health services provided by staff of Rosecrance Berry Center to students of Headstart, Circles of Learning, Fairview Early Childhood Center, Beyer, Kishwaukee, Lathrop, Nelson, Haskell, Ellis, Lewis Lemon, Summerdale ECE, Macintosh & King Elementary Schools.
Services Provided	Services provided by a Pediatric Nurse Practitioner and medical support and direction provided by an affiliate with the University of Illinois College of Medicine. Well-child care: health history, physical exams, sports & health screenings Anticipatory guidance and education for students Immunizations School & Sports Physicals, Daycare & Preschool exams Care for the treatment of minor illnesses and injuries Routine lab tests: throat cultures, hemoglobin, lead screening, TB skin test, urinalysis, & blood sugar Management of chronic health conditions such as asthma, diabetes & ADHD Medication prescriptions for identified health conditions Mental health services: assessment, individual, group or family therapy All Kids Application Assistance
Cost	Clinic accepts Medicaid/All Kids cards, cash and checks, as well as most major credit /debit cards. Fees for patients not covered under Public Aid are determined by a Sliding Fee Scale based on family size and income. We do accept some private insurance such as Blue Cross Blue Shield. Cost is $16.00 each for immunizations for underinsured clients (private insurance that does not cover the cost of immunizations). Payment is due at time of service.
How do I get services?	Appointments available most Tuesdays, Wednesdays and Thursday afternoons and all day on Fridays. Summer hours: Monday & Tuesday all day & Wednesday mornings. Call for an appointment: 815-972-7200.
