High Risk Infant Follow-up Program
WCHD's High Risk Infant Follow-up Program is for babies identified at risk due to a medical condition, prematurity, low birth weight, or drug exposure. Babies are followed until 2 years of age. Developmental screenings, physical assessments, medical follow-up, public health education and referrals for other services, as needed, are provided in the home.
