    Eligibility
    Well Women Clinic: women between the age of 35-64 regardless of income not seeking birth control
    and do not have medical coverage.
    Cost
    Family Planning Services based upon a sliding fee schedule
     
    Well Women Clinic free - with the exception of some lab fees.
    How do I get services?
    For Family Planning call 815-972-7400 for a scheduled exam.

    Pregnancy Testing and contraceptive pick up are on a walk-in basis at the 555 North Court Street location Monday through Friday from 8:30 am - 4:30 pm. The office is closed from 12:00 - 1:00 pm.

    For Well Women call 815-972-7250 to schedule an appointment.
