The survey consists of 22 multiple-choice questions, some with open-ended response areas. It takes approximately ten minutes, but the input we receive from this survey is invaluable. The survey will remain active until at least May 15 (depending on response numbers) in order to allow the maximum number of people to participate.
Even if you did not receive an H1N1 vaccination for any reason, your input will still be of great assistance to us. If you attended a WCHD clinic, you will have the opportunity to tell us how the clinic went, and any suggestions you may have for improving future events. Figures show that the Health Department administered 62,500 vaccinations at clinics and in schools. If even a portion of those recipients fills out the survey, it will go a long way towards improving response in the future.
